Follow the following steps to apply for your TOLL COLLECT cards:

1. If you don´t have UTA cards, fill here your contract.
2. Fill the application forms
"User registry" and "Vehicle registry".
3. Sign and send to Servisa Tarjetas S.A. to the address below.

Note: The formalization of this application will be subject to the approval of our credit department, that can require a bank guarantee or cash deposit depending on the number of cards and the estimated invoicing.
